What every future entry must show

Each entry records: date, reviewer role and credit preference, synthetic scope, finding, project response, remaining gap, and status. The log updates after each completed review and at least monthly while review intake is active; a quiet month records that no new outside finding was completed. Security findings appear only after coordinated disclosure. Declined changes retain the reason.
